Coordinated Services Team
Coordinated Services is a statewide initiative designed to improve the lives of individuals who have complex, and often multiple, needs. Local agencies, as a team, coordinate client services that best meet the needs of each individual. It is a process based on family and community values and is unconditional in its commitment to provide services for each child and family. The principles of the Coordinated Service Team include: a family-centered approach, participant involvement throughout the process, community support service involvement, a strength-based and team approach, promotion of self-efficiency and a belief in growth, learning and recovery. A plan of care is developed with input from the client and all involved agencies. Service coordination ensures that services are not duplicated and that the client needs are being met. Referrals for the Coordinated Services Team can occur through parents, schools, law enforcement agencies, human service agencies, the community or family members.
